Student Health Forms

The Cooper Union requires all students to complete a number of health forms. The required health forms packet includes a personal medical history, physician medical clearance, record of vaccinations and immunizations, disabilities self-identification form, as well as an emergency action plan form.

 

All forms must be completed & submitted by December 13, 2024. All forms received after December 13, 2024, will automatically be assessed a $100 late processing fee. 

 

All Undergraduate AND Graduate Students who are NEW to The Cooper Union:

You must complete all health forms available in the applicant portal / on this website. The Disabilities Self-Identification Form is the only optional form that is not required. 

 

Graduate Students who PREVIOUSLY ATTENDED The Cooper Union:

You do not need to resubmit the meningitis acknowledgement form or the vaccinations form. However, you will need to submit an updated physician’s clearance form, your up-to-date personal medical history (parts I & II), and the emergency action plan form.

UPLOADING YOUR HEALTH FORMS: Health Forms should be uploaded into the applicant portal HERE. When you login, scroll down, under the heading that says "Checklist" select Medical Forms from the dropdown menu, choose your file and click upload. If you are having issues, please email healthforms@cooper.edu AND admissions@cooper.edu

IMPORTANT NOTES: 

  • All forms must be complete & submitted by December 13, 2024.
  • All forms received after December 13, will automatically be assessed a $100 late processing fee. 
  • Students cannot be registered for classes and cannot access any Cooper Union facilities, including our Student Residence Hall, until these forms are completed. Fall semester course registration for new students is automatic but will be withdrawn for students who have not submitted their completed health form packet. Not completing these forms by the required deadline may have implications for course registration and availability. Students will not be permitted to attend in-person courses in the summer unless the entire health form packet has been received and is completed. Late forms can result in course registration complications and may result in the loss of an offer for on-campus housing.
