Incident & Accident Forms
The Incident Report form should be used to report thefts, accidents or other safety problems to the guards.
The Employee Injury Report should be completed regarding any accident/injury and immediately reported to the employee's supervisor.
The Supervisor's Accident Investigation Report should be completed as soon as possible after the accident by the immediate supervisor. It must be completed no later than 24 hours after the incident is reported and immediately sent to Human Resources.
